Employers do not use SAFHIS directly but rely on its results. What matters to employers:
All workers must pass SAFHIS medical screening before permits can be approved.

Employers should ensure their workers attend screenings only at registered SAFHIS panel facilities.

SAFHIS protects employers from hiring medically unfit workers.

process flow

The employer requests an appointment via the digital system. The medical facility receives the request and accepts it.

The worker attends the clinic for clinical evaluation. The primary clinic manages referrals to X-ray or Laboratory partners.

All clinical data is uploaded electronically to SAFHIS. A final "Fit" or "Unfit" decision is generated based on these integrated reports.

The final "Fit/Unfit" status is automatically synced to the employer's dashboard as well as Jabatan Imigresen Malaysia Sarawak (JIMS) for permit processing
FAQs
All workers must complete standard blood tests, urine examinations (including drug and pregnancy tests), and a chest X-ray. Additional tests, such as rectal swabs or serum creatinine, may be required based on the worker's country of origin, job scope, or medical history.

Employers handle administrative tasks like obtaining licences, booking appointments, and bringing workers to the clinic, while medical facilities conduct the clinical screenings and certify the worker's fitness.

No, workers with MSEN licences are excluded from the SAFHIS workflow, whereas workers with SANSOLS labour licences are required to undergo SAFHIS medical screening. Since the MSEN system began phasing out in January 2025, please refer to the attached MSEN vs. SANSOLS Labour Licence guide below for visual examples of the documents and specific requirements.
